Waiver of Liability and Indemnification
In consideration of being permitted to enter and use the facilities at Cloud Nine Play,
I, the undersigned, on behalf of myself, my child(ren), and any personal representatives, heirs, and assigns,
agree to the following:
Assumption of Risk
I acknowledge and fully understand that activities at Cloud Nine Play involve inherent risks of injury,
including but not limited to slips, falls, collisions, and other accidents. I voluntarily assume all such risks,
whether caused by negligence or otherwise.
Release of Liability
I, on behalf of myself and my child(ren), hereby release, discharge, and covenant not to sue Cloud Nine Play,
its owners, officers, employees, volunteers, and agents from any and all liability, claims, demands, actions,
or damages resulting from participation in activities at the facility.
Indemnification
I agree to indemnify, defend, and hold harmless Cloud Nine Play, its owners, officers, employees, and agents
from any claims, damages, or expenses (including legal fees) brought as a result of my or my child(ren)’s
participation in activities at the facility.
Supervision and Conduct
I understand that parents/guardians are required to supervise their child(ren) at all times while inside
the facility. I agree to ensure that my child(ren) follow all posted rules and staff instructions.
I acknowledge that I may be held financially responsible for any damage caused by my child(ren).
Medical Treatment Authorization
In the event of an injury or medical emergency, I authorize Cloud Nine Play and its staff to administer
or seek emergency medical treatment as deemed necessary. I agree to assume full financial responsibility
for any resulting medical services.
Photography & Media Release (Optional)
I grant permission for Cloud Nine Play to use photographs or video of my child(ren) for marketing,
social media, or promotional purposes.
